After successful OTP verification certificate request screen will be shown to you.
Now you have to enter the details like, name, age, date of death, death reported district, name of local body issuing death certificate, etc.
Once the details are entered you have to submit the application form. And an application number will be sent to your registered mobile number.
In the end new certificate as per ICMR guideline will be signed and distributed after district covid-19
death ascertaining committee (CDAC) approval.
